The issue seems to be a problem with autodiscovery failing to update itself in the user's online MS 365 (Exchange Online) account. In this instance, the client had simply defederated their MS 365 users from Godaddy MS 365 services to Microsoft direct. After opening a support case with MS and troubleshooting the issue over several weeks, we discovered that the people experiencing this issue seem to have had something go awry in the migration of their MS 365 mail accounts. Everyone else in the organisation were fine.

Wew were experiencing this issue with a client, where only certain users were having their shared mailboxes disconnecting, after a short while of launching Outlook. I can't try a new Outlook profile at the moment as the user is too busy. I can watch the shared mailboxes disappear and after they are gone I can see under connection status that they are still being sync'd. Yea I've turned off caching and the issue persists. Try to manually add the additional mailbox and see if that helps too. You should also be checking there for whether or not under the advanced tab you have selected 'Download Shared Folders'.

If it weren't showing up at all, I'd say try manually adding the mailbox via Account Settings, Select the Account, More Settings, Advanced Tab.

Give it a shot for troubleshooting purposes on another PC and on the same one with it setup to stream straight from O365. You don't always need to be running in cached mode, and especially if you are using roaming profiles that aren't setup correctly (storing the OST file on the network drive) there can be an awful lot of problems associated with cached. Howdy! Have you tried creating a non-cached new Outlook profile for testing purposes or attached this account to a different computer in a similar fashion for testing?
